A team raises late deliveries. It identifies the real cause as a scheduling handoff, discusses it once, and solves it with a To-Do for the operations lead due in seven days.
The common mistake
Discussing the first thing mentioned. The stated issue is rarely the real one, so spend the time on Identify before anyone proposes a fix.
