Issues List™
A running list of every problem, obstacle, idea, or opportunity that needs to be addressed by the leadership team. There are two: a Long-Term Issues List (anything beyond 90 days) and a Weekly Issues List (what gets solved in the current L10). Nothing falls through the cracks.

The place where problems go so they don’t get forgotten — and where they get actually solved.
What it looks like in practice
During the Scorecard review a number is off track. Nobody debates it. It drops to the Issues List and is taken up later in the meeting in priority order.
The common mistake
Working the list from top to bottom. Pick the three most important issues first, because the team will not get to all of them.
